    The standard pregnancy duration is calculated as 280 days or 40 weeks from the first day of the last menstrual period (LMP), or alternatively as 266 days or 38 weeks from conception. This system, known as gestational age, begins counting from the LMP even though conception typically doesn't occur until about two weeks later. While this may seem counterintuitive, it provides a consistent reference point since most women know when their last period began, whereas the exact moment of conception is usually unknown.

    The Naegele's Rule Method

    The most common due date calculation method, Naegele's Rule, was developed by German obstetrician Franz Naegele in the early 1800s. The formula is straightforward: take the first day of the last menstrual period, add seven days, subtract three months, and add one year. Alternatively, simply add 280 days to the LMP.     Limitations of LMP Dating

    Assumes all women have 28-day cycles with day-14 ovulation, which isn't universally true. Cycle lengths naturally vary from 21-40 days, and ovulation timing fluctuates even in regular cycles. Less accurate for women with irregular periods, recent hormonal contraception use, PCOS, or those who conceived while breastfeeding. Some women experience implantation bleeding mistaken for a period, leading to incorrect LMP dating by several weeks.

    Ultrasound Dating

    Ultrasound dating, particularly when performed in the first trimester, provides the most accurate due date estimates available. Between 7-13 weeks of pregnancy, ultrasound technicians measure the crown-rump length (CRL)—the distance from the top of the fetal head to the bottom of the torso. This measurement correlates extremely consistently with gestational age across all pregnancies during this period, regardless of genetic factors, maternal nutrition, or other variables that cause size differences later in pregnancy.

    First Trimester Ultrasound (7-13 weeks)

    Accuracy: ±3-5 days. The gold standard for pregnancy dating. If dating differs from LMP by more than 5-7 days, most providers use the ultrasound date as it's typically more reliable. Early dating ultrasounds are recommended for all pregnancies, particularly when LMP is uncertain or cycles are irregular.

    Second Trimester Ultrasound (14-27 weeks)

    Accuracy: ±7-10 days. Multiple measurements are taken including head circumference, abdominal circumference, and femur length. While useful for assessing growth, less accurate for dating because individual genetic variation in size becomes more pronounced. Generally not used to adjust due dates unless they differ dramatically from first-trimester estimates.

    Third Trimester Ultrasound (28-40 weeks)

    Accuracy: ±2-3 weeks. Size variations among healthy babies become significant. A naturally smaller baby might measure two weeks behind while a larger baby measures two weeks ahead, both being perfectly normal. Third-trimester ultrasounds assess growth patterns and fetal well-being rather than dating pregnancy.

    IVF & Assisted Reproduction Dating

    In vitro fertilization (IVF) and other assisted reproductive technologies provide the most precise pregnancy dating possible because fertilization occurs in a laboratory on a known date. The due date calculation depends on when the embryo is transferred back to the uterus. For day-3 embryo transfers, subtract 17 days from the transfer date to determine the equivalent LMP, then add 280 days for the due date. For day-5 blastocyst transfers (the most common), subtract 19 days from transfer, then add 280 days.

    IVF pregnancies still experience the same natural variation in pregnancy length as spontaneously conceived pregnancies, but the dating precision eliminates uncertainty about conception timing. This accuracy helps healthcare providers make better decisions about monitoring and interventions. Frozen embryo transfer (FET) dating uses the same calculations as fresh transfers—the embryo's developmental age at transfer determines the calculation, not whether it was previously frozen.

    Term Classifications

    Preterm (Before 37 weeks)

Each additional week in the womb, especially before 34 weeks, significantly improves outcomes. Preterm babies may face breathing difficulties, feeding challenges, temperature regulation problems, and developmental concerns.     Early Term (37-38 weeks 6 days)

For this reason, elective deliveries (inductions or cesareans without medical indication) are generally not recommended before 39 weeks.     Full Term (39-40 weeks 6 days)

    The optimal time for birth, associated with the best outcomes for babies. Full-term infants have fully mature lungs, better temperature regulation, more efficient feeding, and superior neurodevelopmental outcomes compared to early term babies. The last few weeks of pregnancy involve crucial brain development, lung maturation, and accumulation of body fat for temperature regulation and energy reserves. Unless medical complications necessitate earlier delivery, waiting for spontaneous labor in this window provides babies the best start.

    Late Term (41-41 weeks 6 days)

    Pregnancies extending to 41 weeks remain within the normal range, though healthcare providers typically increase monitoring frequency. Additional prenatal visits may include non-stress tests and amniotic fluid checks to ensure the baby remains healthy. Many providers discuss induction options as pregnancies approach 41 weeks, particularly for first-time mothers, balancing the small risks of continuing pregnancy against intervention risks.

    Postterm (42 weeks and beyond)

    Pregnancies continuing past 42 weeks carry increased risks. The placenta may begin functioning less efficiently while the baby continues growing and requiring more oxygen and nutrients. Risks include meconium aspiration (baby passing stool before birth), oligohydramnios (low amniotic fluid), macrosomia (very large baby), and stillbirth. For these reasons, most providers recommend induction by 42 weeks, and many suggest earlier induction to avoid reaching postterm status. True postterm pregnancy (as opposed to incorrect dating) is relatively uncommon.

A landmark 2013 study published in Human Reproduction examined women with regular cycles and known conception dates, excluding premature births and medical interventions. Researchers found pregnancy length varied by up to 37 days, with multiple factors influencing duration.

    Managing Expectations Around Your Due Date

    When Medical Intervention Becomes Necessary

    Healthcare providers monitor pregnancy carefully, especially approaching and passing the due date. Various complications may necessitate earlier delivery. These include preeclampsia (dangerous high blood pressure), placental problems like placenta previa or abruption, intrauterine growth restriction (baby not growing appropriately), gestational diabetes poorly controlled by diet and medication, decreased fetal movement, and certain maternal health conditions like poorly controlled diabetes or high blood pressure.

    Conversely, continuing pregnancy significantly past the due date requires increased surveillance. Most providers begin offering induction around 41 weeks, with strong recommendations by 42 weeks. Research comparing immediate induction at 41 weeks versus expectant management (waiting with monitoring) shows slightly better outcomes with induction, including reduced cesarean rates and no increase in intervention complications.
